Parent Input Forms

We are approaching the time to put together class lists for the 2012-13 school year.  If you have any important feedback to share with the principal in the areas of academics, social skills, physical needs or emotional needs of your child that would assist in the best placement in a class for next year, you may pick up a Parent Input Form from the office.  These forms will be available from March 19^th thru March 30^th.  PLEASE REMEMBER— THESE FORMS ARE NOT TO REQUEST A SPECIFIC TEACHER.
